Thank you for your query to enrol with our practice, please complete an enrolment form and new patient health questionnaire then file with our receptionist, in person or email to reception@nhmc.co.nz. With your permission we will also request a transfer of medical notes from your previous GP.
You are eligible to enrol if you are a New Zealand resident or hold a 2+ year work visa, a driver licence is not an acceptable form of proof of eligibility.
Examples of eligible documents include:
- NZ Passport
- NZ Citizenship
- NZ Residency
- NZ Permanent Residency
- NZ Work Visa (duration of 2+years)
- NZ Birth Certificate plus 2 forms of supporting identity information:
- one with a photo, this ID can be your NZ driver licence and a letter addressed to you at your current address (not required if your passport is provided).
Please note, we are unable to register you at our practice unless all the above information is received, and verified.
